You can get your child’s immunization certificate at your pediatrician office.

*School-Agers do not need to provide a FL certificate of Immunization, as this is already a requirement of the Indian River County School District.

The VPK Certificate of Eligibility form is only needed for our VPK Students and will be given to you  by the Early Learning Coalition.
